How to File a Claim
Our straightforward process puts you first
- #1Log in to your account or open the daily health app and go to Claims
- #2Select the type of claim (medical, travel delay, baggage, etc.) and complete the claim form
- #3Upload all required documents—medical reports, receipts, boarding passes, police reports as applicable
- #4Submit your claim. You'll receive a confirmation and can track status in the app or portal
Required Documents
To process your claim quickly, please have the following ready when you submit:
For Medical Claims
- Completed claim form
- Medical reports and discharge summary from the treating facility
- Itemized bills and receipts
- Proof of payment (bank statements, credit card slips)
- Police report (if applicable, e.g. accident)
For Travel Claims (Cancellation, Delay, Baggage)
- Booking confirmations and cancellation notices
- Boarding passes and flight delay confirmations from the airline
- Baggage claim tags and Property Irregularity Report (PIR)
- Receipts for replacement items or additional expenses
Our claims team may request additional documentation depending on the nature of your claim. We aim to process complete claims within 14 business days.
